In an exciting new study, researchers have pinpointed essential elements that significantly boost the caliber of care delivered by home care cooperatives, collectives owned and managed by home caregivers themselves.

This illuminating research, published in the esteemed journal Health Affairs Scholar, revealed four primary drivers of improved care quality at cooperatives, all revolving around the empowerment of workers as stakeholders:

  • Empowerment through input: Engaging caregivers directly in care planning amplifies the relevance and adaptability of services provided.
  • Encouragement from co-ownership: The sense of ownership derived from being co-owners of the cooperatives significantly propels worker motivation, leading to enhanced care quality.
  • Improved talent acquisition: Cooperatives prioritize recruiting top-performing workers, raising the overall standard of care.
  • Superior training: Providing thorough, hands-on training equips caregivers with the tools they need to deliver superior care.

As Dr. Geoffrey Gusoff, the study's lead author and assistant professor of family medicine at the David Geffen School of Medicine at UCLA, notes, "These care-enhancing practices, as identified by participants, represent testable interventions that have the potential to significantly improve care quality across the home care sector."

Traditional home care services often struggle with high turnover rates, limited training, and communication issues. To address these concerns, embracing the cooperative model could pave the way for improved home care quality, bringing benefits not only to care recipients but also to the broader healthcare system.

Previously, the research team focused on how cooperatives can reduce employee turnover through methods such as better compensation, fostering a sense of community, and offering control. In this study, the researchers delved into the cooperative practices that appear to boost the quality of care.

To gather insights, the researchers carried out 32 semi-structured interviews with home care workers and other staff at five cooperatives. However, some limitations were acknowledged, such as a lack of input from care recipients and non-English speaking workers. It was also noted that there may be selection or recall bias in participant responses, and further research is required to measure the actual impact of the identified drivers of care quality.

Moving forward, the team plans to conduct quantitative studies to assess how these factors influence patient safety, experience, and health outcomes[1]. Additional authors of the study include Miguel Cuevas and Dr. Catherine Sarkisian of UCLA, Dr. Madeline Sterling of Weill Cornell Medicine, Ariel Avgar of Cornell University, and Gery Ryan of Kaiser Permanente Bernard J. Tyson School of Medicine.
